what is needed to do 4 lug to 5 lug swapout

Ok guys I need some help. I havent been able to get work on my car due to cancer. My vehicle took a heavy loss of detail parts and degreadation while I tried to get it repaired here in Eugene. I have not been able to find wide wheels with 4 lugs and Evo wheels can be mounted on VR-4s. What do I need and does anyone have necessary parts that they are not using or removing from car being parted out?
