Looking at the pressure plate side of the clutch. Where did you come from, little piece of metal? Could one of these blown out covers have come off? No, they're all there... What about down here, holding the springs in?

That's one of the main reasons I don't recommend a sprung hub on a "puck" clutch. The engagement just tears them apart. Go solid hub, it's lighter, and you'll never have that again.
